I've just received a reminder but I filled in the original form, why have you done this?

It may be that the forms have crossed in the post or we may not have received it in the office. The easiest thing to do is ring Electoral Services to see if it has arrived or simply complete the second form and send it back to us, we would prefer to have received two forms rather than none at all.
